Michael Hill's suggestion is a key success factor.

Having the team co-locate and pair promiscuously at the beginning of the
project will make a tremendous difference.  This is when the team gels and
also when the team attains a shared mental model of the problem space and
the solution space.

Even just 1 or 2 weeks will make a big difference.
