Hello,
An update. The key to starting anyone new to forming xp team is the willingness of its members to go through trials and errors of getting started. Aside from having an extroverted and initiative-taking mindset (also see Working Together Apart PDF), it's best with people who know something/anything about pair programming and extreme programming to not feel like total fish out of water. It's even better when the members know each other as friends and trust each other at the start, feeling more at east when things get tough or unclear.
One of the simplest solutions that I found from this experience of setting up a remote pair programming team is to hire right. I am not a programmer. More a user experience designer who knows how to project manage. There's no way I can get the experience that I need. What people like me do is dive into the sweat and guts of things, hoping that by the second and third time in learning how to drive, that I'm better than when I first tried. Failure is an option when the costs are low and at the start of a project. As xp practitioners know, the costs only go way up closer to the end.
So I hired a new team. I looked for someone who knew something about 'pair programming'. Then I asked if she knew anyone else she could recommend to work on a team. She gave me two other names. We spoke. The next phase up is to test them on small projects with baby steps. The next three months is the real test of how a team works together. Have no doubt, there will be communication problems. But with a documented system and constant communication (almost daily, if not frequent video and audio chats and emails), those differences become less.
That's my update. Next up is setting up the infrastructure with the team. I know there are tools out there. I just have to try a few of them and ask the team what they think. What's more helpful, we need to do a few more projects to run a fluid first, second and third test drive. I'm glad I discovered XP as a user experience designer. I'm a bit saddened to think that most people -- including developers -- never heard about it. It fits so well as an extension of the UX design process. At least for me, I'm glad I have the vision to see things through when others don't.

I have had pretty good success as a coach facilitating Scrum teams to discover the need for engineering practices in order to meet their responsibility and then adopt much of XP. Over the years I have gotten a lot of pushback on this very group about not adopting XP from the beginning, but I believe the team continues with XP more successfully when it was their idea instead of mine. The team also learns inspect and adapt before I have to move on, because there are actual problems to learn from and fix.
That has worked pretty well for co-located Scrum teams and even partly co-located teams (ones where less than half the members are remote). Maybe it is just me, but I have not had anything but failures facilitating the transition from vanilla Scrum to XP with fully distributed work groups that were not already a mature team.
So, for fully distributed work groups, I now believe adopting/training XP from the beginning is the better tradeoff (while still preferring the Scrum to XP transition for co-located or mostly co-located teams). This means mandating pairing, TDD, group code ownership, CI, etc. from the beginning. I believe this is the right tradeoff, because it increases the chance that the group will gel into a team despite the lack of face-to-face interactions, and even if that never happens, the XP practices will protect some of the value of the work.
