These are the ones I know of:
  Linux: cachegrind (in valgrind), oprofile (, vtune)
  Tru64: atom tools; prof, uprofile, hiprof, pixie
  Mac OS: chud
  Windows: vtune

AFAIK, most of these provide raw data that you need to know how to
interpret.
