This is because Quality Practices are additional requirements that aren't 
listed together with the other requirements, and when Quality Practices 
are taken for granted, they don't look like waste, because they look like 
they are simply part of the Process because it's the way people believe 
that it _has_ to be done.

If you start seeing them as part of the requirements, the "almost/barely" 
line goes back in its place, which is where/when requirements are met.

There's always a rephrasing that puts that line at almost/barely because 
there's always a way to look at "barely", "almost" and "is working" that 
will result in that very pattern. For example, in this case, it was the 
"requirement" vs "quality practice" split that had to be merged, and in 
another case, it'd be something else.
