Robert Glass is my favorite author in that regard. Software Runaways
and ComputingFailure.com are two books of his that are more recent. I
think I have all his books, and can recommend them all. Brichter's
'The Limits of Software' also comes to mind. Steve McConnell also
talks about it in his books.

I can't cite case studies OTTOMH, but most references I've seen
address the massive systems that have gained public notice due to
their spectacular level of failure. e.g. the infamous FAA system.
