BigIP LTM Default persistence failing

Issue: BigIP LTM only using fallback persistence
Unit: 1600
Ver: 9.45

I have a VIP set to use default persistence as Cookie and fall back to
use source address. Cookie method is 'HTTP Cookie Insert'
Statistics are showing the persistence mode of Source Address Affinity only.

Am I missing something?
