The actual format/layout will depend on your own HR dept's wishes; without that, it is impossible to do as you ask, I would suggest. Whatever the format, the content should be based on the Scrum Guide description with the addition of certification (CSM or PSM (I or II)) and whatever previous experience you desire. Simples Steve Ash
