NASA has been using multiple systems programmed by team that do not
know each other or share code on all(?) missions at least back to Apollo.

There was a shuttle launch that was scrubbed because the computers disagreed.
That was traced back to poor wording in the program specs.
The spec used a colloquial phrase that meant different things in different parts of the usa.
