Programmatic usage of 2.2

Hi Jonathan,

we've been trying to use 2.2 in our setup, but it seems that the
interface for programmatic usage has changed in 2.2.  

1) Can you tell us how the "write" (etc) methods are now used on the
Response object?

2) And, how and when is its "out" attribute used?  (It used to be a
string, seems to expect a file-like thing now?)

3) It seems to also now expect QUERY_STRING in the Request's environment
which previously was not necessary.
