As the cable modem always sits between the cable coming out of the 
wall and whatever is connected to the other end (PC or m0n0wall), it 
should make no difference. As far as I understood, it works perfectly 
with a PC connected to the modem, but not with the m0n0wall (which is 
basically a PC as well, just not running Windows/Linux/MacOS but 
FreeBSD).

It could be a rare hardware issue, where the modem's NIC doesn't play 
well with the m0n0wall's NIC. I've seen such an issue which could be 
remedied by first turning on the m0n0wall and then turning on the 
modem. If the modem was turned on with the router connected but turned 
off, it would hang until power-cycled.

Many years ago one ISP also sold packages which allowed connection of 
one single PC only. It was assumed that this might have been enforced 
by checking the sequence number of packets. Which is probably a most 
ridiculous idea nowadays, with boxes rewriting sequence numbers.
