There are several tables. Each table can have several chains (both built-in and user-created). You're looking at one of the chains in the 'filter' table which is the table you get by default if you don't explicitly specify one using the '-t' option. The CUSTOMFORWARD chain is created by the IPCop package, for IPCop users to use for their own custom rules.
