some doubts about CVSS Score and Risk Factors

I have some doubts about CVSS Score and Risk Factors. In some plugin's description The Risk Factor is High, Medium or Low, and in other the description contains CVSS score and CVSS vector and in other, the description contains both. Why this difference between plugins? I have one more question In a result.nsr, the condition of NOTE or INFO what does they imply? Thanks you in advance -- Ulises U.
