I think this is the "Trusted Software Development Methodology", published 
by the Department of Defense Strategic Defense Initiative Organization. 
The document number is SDI-S-SD-91-000007, dated 17 June 1992.  If you 
can't find a version of this online (there's two volumes), I have a paper 
copy on my desk and a copy I just scanned on my hard disk.
