I'm a fan of AuthAnvil by Scorpion Software http://www.scorpionsoft.com/agents/web/. They support hardware tokens and soft tokens.

What do you mean by out of band though?

We have used Portwise for our 2-factor authentication requirements (although have not used it for securing outlook since it was not in scope for compliance)..However have found it to be quite easy to setup manage and implement too. http://www.portwise.com
