And there is always the buried code that can be a problem.  For example, 
a hidden
process that checks for USER1 to exist & is enabled.  If not, then 
elevate USER2's
rights at a certain time of day, and decrease before the scheduled daily 
audit runs.
User2 being either some sort of service profile or a user profile for 
someone who
rarely ever signs on.

I've seen something along that lines a long time ago.
