An exclude_dir mask built with the string "foo" will match all string
that represent a subdirectory (or sub-file) under "foo", including "foo".

This if what is said in the tutorial (paragrah 4) and in the reference
manual for the API, and it stays true. So of course, don't get confused
by the name, to exclude the given "foo" directory and any file or
subdirectory in it, you need to negate this mask with a not_mask. The
name comes from the fact it is used to exclude directories, but you can
use it at your will to include directory, for example.
