Exclude / Include / Dir -- Additive would be nice!

A couple requests:

1) I too ran into the MAXPATHLEN thing (just try excluding your Mozilla Firefox
Cache directories) -- is there no way to change this other than the source?
2) Couldn't the Include / Exclude and Dir lines be additive?  (Rather than just
the last one being used)  This would fix the MAXPATHLEN thing, since it wouldn't
all be on one line.  It would also be *great* for inheriting!  For example, I
have to redefine my global excludes inside a server item, because I had to add
an exclude.
3) Could it be documented in the man page that commas are used for separating
items on the same line?  I couldn't find what was used to separate it...
