monitor mode comes from a time before the "distributed PMNS" when you 
needed to install a local PMNS to make metrics available for clients 
(aka monitors).  For a long time now, the PMNS has been delivered via 
pmcd or archives, so I cannot think of a reason why anyone would need or 
want to choose "monitor" mode.

Since there are only a handful of people on the planet that remember 
these early days of PCP, I think we could safely drop the whole thing 
... this would cut some code out of pmdaproc.sh, remove the confusion 
and trigger a massive QA re-base (almost 100 tests would need to be 
reviewed and some changed and all the .out files remade), but that would 
be a once-off effort.
