Hi Jethro,

I can reproduce the issue with `columnsets-004.tex`
(https://github.com/contextgarden/context/blob/a0ba34ef964f40fe3d5045437cdc1b48ba6feffd/doc/context/sources/general/manuals/columnsets/columnsets-004.tex).
The second columnsetspan doesn’t show.

Sorry, Mikael or Hans, could you take a look at it (since it might be a
regression).
