second release candidate for 1.9.19

Hey all,

Here is a second release candidate for latex2rtf.  This one seems solid.
If there are no serious regressions then I hope to release 1.9.19 on
SourceForge in the next few days.

r811 add missing .bib file to tarball.  Update changelog.

r812 Improve \nolinkurl{} so that it typesets correctly and works with
      '%' in the url. Fix long-standing issue with user-defined
      environments and scope.  Modify unknown command message...again.

r813 Put \- hyphenation command back in. Originally removed before I
      realised the problem was with OOwriter, not with rtf generally. It
      appears to work fine with MSWord.

r814 Increased the stack from 2000 to 10000. This was motivated by the
      failure of longstack.tex to parse properly. The increased stack  
size
      fixes this for now, but I suspect there is a problem with the  
stack
      not getting emptied.

r815 Evidently url.sty and hyperref.sty work differently.  Implement
      accordingly.  Revert linux.tex to use the url.sty package and let
      hyperref.tex test the hyperref package.

r816 WH: Updated documentation reflecting successful testing under
      Vista.

r819 WH: documentation: Tables section (-t2 option) and editorial

r821 Fix for missing paragraph break for a \url{} that starts a new
      paragraph.

r822 Change how \, is translated.  Make it a small non-breaking space
      instead of a just a small space.

r823 CmdNoBreak() was so good it turned out that it was already in the
      codebase as CmdNonBreak().  Eliminated duplicate routine.  Added
      support for \thinspace

r824 Use \charscalex instad of \fs to change the size of a non-breaking
      space.

r825 Update ChangeLog and version.h for release.

r826 Fix for regression in formatting section numbers

r827 Last fix only improved formatting of section numbers.  This one
      removes the extra paragraph start and eliminates a long-standing
      hack in SetTexMode().

r828 Eliminate long-standing bug with spurious spaces before a
      paragraph.  This is still not quite right, but it mostly gets
      paragraph indentation correct.  Currently, paragraph indentation  
is
      inhibited after an environment closes...which of course is wrong.

r829 refactor all the vertical mode routines into a single file.  No
      functional changes

r830 Add the new files

r831 update the Makefile and reduce number of header files used by
      vertical.c

r832 remove a couple of global variables

r833 eliminate global paragraph alignment variable

r834 rename CmdStartParagraph to startParagraph() because it is not used
      like the other CmdFunctions()

r835 Change accessors for the TeX mode.  These now follow the format of
      the other functions in vertical.c

r836 rename ANY_INDENT to GENERIC_PARAGRAPH and other for clarity.  Fix
      the indent bug for paragraphs following a verbatim environment.
      Make 'references' bold in APA style.
