installing package automatically each time I compile

Hello all,
I wonder what I did wrong that causes miktex to start the package manager to install some packages (e.g., ifxextex) on the fly *every time* I run pdflatex.  Luckily disabling the install on the fly option solved the problem, but there is surely a better way. Any thoughts are welcome.
