miktex "observes" its own packages and "repairs" them when files in
this packages have been changed - this doesn't neeed to be fatal
changes. I think change dates or some file attributes can trigger
the "repair".

If you have a multiuser installation run the update manager in both
modes (admin and user mode).
If you have a network installation check if you have installed
things in a roaming profile.
