Sorry, but do you really believe that someone can help you if the
only information you give is "we are using passivetex"?

Make a test document that demonstrates the problem. And describe how
you are compiling.

I could run "pdfxmltex teiu5.fo" with the .fo from here
http://projects.oucs.ox.ac.uk/passivetex/index.xml?ID=body.1_div.2
without any problems.
