It is parsing the mathdesign maps. 

1. If you run a small hello world example with pdflatex (!) does it
use this map (check the log-file):

C:\Documents and Settings\Maurizio\Impostazioni locali\Dati
applicazioni\MiKTeX\2.9\pdftex\config\pdftex.map


2. If you run your example with 
 \usepackage[utopia]{mathdesign}
with pdflatex does it work?


3. If not, does it work if you add
 \pdfmapfile{=mdput.map} 
to the document?
