2.99.1: new beta / almost release candidate

Well, shortly after 2.99.0, we now see this 2.99.1 release. Besides
the fact that I still want to try to rewrite texconfig, this can be
considered something like a release candidate.

I have cleaned up a lot and most things are in a state that I feel
comfortable with.

Since a large amount of C code has changed (gd lib, ncurses, dvipng)
I'd be happy to see a lot of testing and feedback (positive and negative).

Changes in the src area:
        * config.guess and config.sub updated
        * minor changes to tetex.ac / set enable_tetex_build for the
          upcoming lcdf-typetools (not (yet?) used by teTeX, but TeX Live)
        * teTeX-FAQ cleaned from obsolete entries and added an entry
          about moved map / enc files
        * another improvement in one of updmap's messages...
        * texconfig now redumps all formats after changing
          pdftexconfig.tex
        * upgrade gd lib (2.0.23 -> 2.0.28)
        * upgrade ncurses lib (5.3 -> 5.4-20041016)
        * upgrade dvipng (1.1 -> 1.2)
        * add missing texk/web2c/tests/xampl.bib (for "make bibtex-check")
        * changes to INSTALL, TETEXDOC.tex
        * Makefile.in: set up "make check" for texmf-dist

Changes in the texmf area:
        * regenerate manuals in doc/programs
        * add esint / mh bundle
        * copy thmtest.tex from source to doc
        * rm doc/latex/amscls/{amsrefs.dvi,textcmds.dvi} (keeping only
          the .pdf versions)
        * update doc/help/ctan/CTAN.sites
        * update m-bib (for context)
        * update supp-mis.tex (from Hans) to fix a clash with layouts.sty
        * update thumbpdf (cygwin fixes)
        * update l2tabuen.pdf
        * update Catalogue
        * update doc/index.php and doc/texdoc.php + friends (from Joao
          Palhoto Matos) and doc/index.html
