Okay, this is taking a little longer than I expected.  I started
reviewing the manual and got annoyed with the lack of proper indexes and
the redundancy of the copyright notices and so on and couldn't help
myself but take a swing at those issues first.  Our manual should make
use of all the nifty features we support, after all.  :)  After that I
reviewed and rewrote up through section 4.3 or so.

I've checked in your changes plus what I've altered so far.  I figured
that is much more in the spirit of CVS development anyhow since if you
feel the need to rewrite or add to anything then I won't be preventing
it.  I think I will need another few weeks to get through the whole
manual since I am just doing this in my spare time.

Pat, if you wouldn't mind looking at the questions I've been leaving in
comments in the manual, I would appreciate it.  I've annotated all of my
questions with my initials (`DRP').

Finally, is there a good reason for the custhtml.texi and custpage.texi
sections being kept outside the texi2html.texi file?  Automake doesn't
know to update version.texi when those include files change.  It would
be easier to combine those files into texi2html.texi than to get the
Automake bug fixed, but maybe it is worth it?
