adds breadcrumb navigation, i.e., 


   top / level1 / level2 / ...

by adding some code to the texi2any init file `lilypond.init`
(together with some CSS changes, see attached images for results).  I
wonder whether the Perl code is sound, so please have a look and
comment if you have some time!

Irrespective of that it would be great if Texinfo could provide
breadcrumb navigation natively...
