I had to fix the patch to include it (delete a stray <bridgehead> line)
but it's fine otherwise.

I do get a few errors from jade:
- The <anchor> tag isn't allowed where it was.  I've added the "id" to
  the <bridgehead> tag.

- This gives an error:
	<entry>
	<variablelist>
  But this works:
	<entry><variablelist>
  Don't ask me why!

- You added links to "ref-dll", "ref-program", etc.  That should be
  "cmd-dll", "cmd-program", etc.

It's not too bad, but we should try to finish this XML vs SGML
discussion some day to avoid these problems.
