To make a single-file HTML doc, you will want to reference the docbook.xsl 
file in the same directory.  If you want to get really wild, you can use the 
changebars.xsl stylesheet and add the revisionflag attribute to tags and 
give it a value of 'added', 'changed', or 'deleted' and it will 
appropriately mark the section enclosed by the tag.  Looks pretty neat, but 
probably a lot more than you need :)

Sorry, I forgot about that.  The DTD archive is at 
http://www.oasis-open.org/docbook/xml/4.2/docbook-xml-4.2.zip

I just did a trial and created an empty directory and copied in the DTD 
files and the docbook-xsl files and my document, and I was able to convert 
it to HTML and chunked HTML with xsltproc, so these should be all the files 
you need for that.  You will need an extra tool to convert to PDF, though, 
since xsltproc can only create the FO file.
