Add Contents dialog: No longer shows files in .cvsignore by default - 
there is
now an option in Preferences to get the new behaviour (RFE [994412]).

Commit dialog: There is now an empty entry at the top of the History 
dropdown,
so that it is easy to select the previous entry using the mouse. (Torsten)

Make Patch: Do not show "Browse" button when only files are
