Sorry for the delay in response.

We did some checking and discovered that all affected files we have found have a blank line (new line character only) as the first line in the file.

I created a sample project that demonstrates the issue.  cvs annotate gives the correct output when run against test2.c but, ViewVC seems to ignore the first line when it is blank.
