Check the protection table (p4 protect).  Don't change anything
yet, just look at it and see if you can find the line(s) that should be granting
him access.  Then compare that to everything that you know about how
he's connecting.  In particular, if you have a version of that P4V
screenshot that doesn't have his username blacked out, look VERY carefully
at the username and compare it character-by-character to what's in the
protection table.

Him reformatting his computer would not have changed the protection table, so
the puzzle here is to figure out what did change.  My guess is that
the user name is different -- maybe only in case?  (Is your server on
Unix?  If so, user names are case sensitive.)  The fix is
for him to set his Perforce user (on his end) to match the name specified in the
protection table and/or group(s).

I say "don't change the protection table" because odds are good
that if you just change the protection table to match his new username, it's
going to be out of sync with something else (like, his workspaces are no longer
owned by the same person, his change history is going to split at the name
change, all sorts of annoyances).  It's easier to fix the one
thing that changed than to slowly change each of a dozen other things to try to
match it.   :)
