Unable to see the Tortoise in Windows Shell Extension during debug

Hi I would like to know the steps involved in order to debug BOOL CTortoiseProcApp::InitInstance(). I followed debug-hint.txt. After I run the register.reg file pointing to debug code, I don't see the Tortoise SVN in the windows shell extension.
