Server randomly duplicating users?

We keep running out of licenses in the SOS admin tool.  When I audit the license list, I am finding several of our users duplicated...  I am only admin - so this SEEMS to be a bug to me.  But it is strange that we are just now seeing this behavior after 3 years of use.
