v4l error handling

I suggest tha the error handling is improved in the v4lmodule and the
vloopback output driver in the following way:
whenever a error occurs while reading from or writing to
the v4l device, the device is closed and reopened.

Reason: when using vloopback and one of the involved processes
closes his part of the v4l pipe, the other part of the pipe
must be closed as well before it can be used again.

Another reason: when using usb cameras, it could happen that
the camera is unplugged and plugged in again during operation.
(I tried this yesterday with Martin's usb cam and the pwc
driver crashed, had to reboot to use the cam again...)
