mpeg2enc pass-2 quantisation bug fix

Hi all,

I've just commited a change that should (hopefully) fix the problem 
that occasionally caused recoding of frames in the second coding pass to 
produce streams systematically exceed the specified maximum bitrate.

Thanks to Bernhard for passing on some nice samples that demonstrated the 
problem for debugging.

Next stop - those valgrind issues...
