I applied your patch and tried it. One thing is, I get lots of buffer 
underflows:

[mpeg1video @ 0x83d5a28]rc buffer underflow0086/90086   0%  0%  0.0% 0 0 3%

I also increased the gop size to reduce the blocks. I tried

lavc_venc_context.gop_size = [2,3,4]; // I-only

but it didn't change anything. cmd was:

  mplayer -vo mpegpes -ao mpegpes -vop 
lavc=9000:25,expand=688:576:-1:-1:1,scale=688:404

Can the buffer overflows be the source of the blocks?
