v 8.4 again

The new chapter menu is very nice. It might be better to call the
indexes with a prefix like chapter, title etc. since the menu is so
shotr with only numbers.

Moreover, when I open the menu on top of the playback window, it flashes
a lot. Does the output window redraw in response to redraw events?
Wouldn't it be sufficient to do this only when playback is paused?

Secondly, I really haven't had any time to dwell by the gui code, so I'm
rather asking; would it be hard to make the gui window pop up on top of
the playback window on a right mouse click?
