Ok, another thing you will notice when Xv is not working is that
when you try to fullscreen the video it will not fill up the screen.
There will just be black borders around the picture.

How to enable/get Xv working depends on what gfxcard and what version of
XFree86 you are running.
Some of the gfxcard manufactureres have their own binary X drivers that
you must use to get Xv. Some cards only work with a new version of XFree86
(4.3.0). Check the ogle faq for some more info on this.

you still have <resolution_src> set to user

This tells that there is no suitable xv adaptor present.
