I think this is a problem/bug in the xserver.
I have noticed this problem myself on my matrox g200 card after I
upgraded xfree86 (don't remember which version it started in).
When using Xv the mousepointer sometimes leaves a black square in the picture
when going from normal to fullscreen (also happens with mplayer).
This started with the version of xfree that introduced the
mouse "shadow" and automatic blanking of the mouse in Xv windows I believe.

Going from/to fullscreen usually removes the black square,
