To answer my own question.  I was able to compile everything and have it 
work with CTL1.4.1 and OpenEXR_CTL 1.0.1.  The problem was that cmake 
defaults to debug builds when running through makefiles and that was 
creating DLL hell with msvcp100.dll and msvcp100d.dll and msvcr100.dll 
and msvcr100d.dll in the same executable.
Sorry for the bother.  I hope this helps others.
