As you say, TwinView works by tricking the X server into thinking that
both displays are a single big screen.  It's supposed to also provide
fake Xinerama hints so that Xinerama-aware applications and window
managers can find out where the virtual boundaries are between the two
monitors.  It's possible that this Xinerama information is either
disabled or not being used.

Try maximizing a window.  It *should* only fill the one monitor that
it's on.  If it ends up covering both monitors, then the window
manager is having problems recognizing that there are two monitors.

If /etc/X11/xorg.conf contains a "NoTwinViewXineramaInfo" setting of
"true", then the TwinView Xinerama data is disabled.  There are a few
driver settings that can control how the Xinerama data is presented,
for example see:

When using TwinView's fake Xinerama I think you also need Xinerama
disabled at the server level, otherwise the server's own Xinerama code
could interfere with TwinView.  So xorg.conf should *not* contain:

  Option "Xinerama" "true"

It's been a few years since I've used TwinView, so it's also possible
that this information is out of date.  NVIDIA supposedly released
several changes to TwinView and XRandR support over the past several
months, so if you're using a really recent version of the driver some
of the configuration options have changed.
