dialogs and 481 on bye

Hi,

I think that may be a leak in the dialog table when receiving 481
responses on BYE.

Steps to reproduce:

1.       Make a call from a nist-sip based UAC to any device (in my case
a cisco 7905 phone)

2.       answer the call

3.       disconnect the phone's power cable

4.       reconnect the power cable

5.       disconnect the call on the UAC side.

 

The remote endpoint sends 481, but the dialog remains in the dialog
table forever.
