"start-index" on Blogger API v3.0

I recently migrated to the v3 blogger api, however, it seems that with this 
api we can no longer pass a "start-index" as a URL parameter to retrieve a 
list of posts offset by the start-index. I would like to always fetch 10 
posts while also having the ability to define a start-index. Currently, we 
can send a pageToken to be able to fetch the next set of results. Is there 
a way to fetch the "5th" page of results (assuming always fetching 10 
posts/page) without a pageToken? Or, is the pageToken generate in a 
reliable way that I can programmatically create it and attach it to my GET 
requests? Any help on this matter is greatly appreciated. Thanks.
