shortcuts to open links in new tab and new window

Currently it seems like you can only have it one way, via the
"middle_click_opens_new_tab" dillorc option. (And to do it the other
way, you have to right click and navigate the menus. A lengthy
process :). I think it would be quite useful if both were available via
a shortcut... perhaps have this option toggle the meaning of
ShiftMiddleClick and MiddleClick... whereby one will open the link in a
new window, and the other in a new tab?
