FastCGI incomplete headers

Hi , i get this error , and don't know how to debug , 
i got apache 1.3 and php5 over fastcgi with 8 Vhosts , now im trying to
create new vhost with same template config like others but i got this
error permanently and when try to access new vhost there is only blank
screen.
in error_log i get :

 dont know what to do. i can post my cofigs if wanted.
