Intermittent failure to load [#include]

We're getting a strange error. After our webapp has been running for a 
while, a week or two, we get an error that looks like this:

[ERROR: Error reading included file /foo/bar.ext Template /foo/bar.ext 
not found. ]

We're loading the file using [#include]. Up to this point, it loads the 
[#include] just fine. Once we get the error it does not recover. If we 
restart the server it works again.

I have a custom template loader, but it's a pretty simple 
fetch-from-disk loader. Everything else is plain vanilla, including the 
template cache, etc..

We are using a custom servlet for processing templates.

I thought the error would have to be in our custom code, but a few weeks 
ago we started to get the identical error, and we were using a totally 
different template loading mechanism and servlet mechanism. I did a 
total system rewrite between the two incidents.

What's hard about this is that we have no easy way to reproduce the error.

The logs show nothing.

Has anyone seen anything like this? I know I'm not giving enough 
information for you to diagnose it, but I'm just hoping that someone has 
seen it before.
