When the toolbar implementation was changed to take the new toolbar editor into use we might have had to reset the toolbar layout back to defaults or something. We probably didn't advertise the change much. Can't recall. In any case you can edit the toolbar from Edit/Toolbar. The spinner visibility is a "feature" of the icon theme, some spinners do not have the "resting" state and that's why it disappears when nothing is being loaded. You can pick another icon theme with better spinner, or create your own spinner.
