When I have 2 messages open, the only places that differ are the arrow
part of 2 the 2 drop downs (not sure if they're even present if you
don't have multiple accounts and multiple sigs) and the title bar.
Everything else is pixel identical.

You can differentiate the title bars, I'm not saying it's some kind of
impossible feat, it's just that every patch has lessened the difference
between active and inactive window titlebars. And if your focus is on,
say, the message part of the mail the title bar's not the first thing
you see.

But when you look back to your screen after taking a phonecall or
looking at a different comp and there are a couple of ichat windows
open, itunes, and a mail message it takes a few seconds to figure out
where letters will show up if you start typing. That wasn't the case
in OS9 or in early days of X.
