Go to Preferences > Advanced > Downloads. Uncheck "hide file types opened with Opera" and in the Quickfind box enter in "rm" (or if that doesn't show up, try "ram"). Highlight it and click "Edit." For the Action, you can have it open with the default program if you have RealPlayer set as the default player for those file types on your system. Otherwise, choose "open with other application" and then navigate to and select the RealPlayer.exe application. OK out. Realplayer should open and attempt to access the file when you click that link. I don't remember whether there's a RealPlayer plug-in for Opera, but if there is you would be able to see it on that preference pane and you could also select that.
